  • Please do not attend your in-person visit, and contact your clinician,  if you have been diagnosed with COVID-19 or had a positive test in the last ten days OR if you have any of the following new symptoms:

    1. Cough

    2. Shortness of breath

    3. Fever

    4. New loss of taste or smell

  • If you can't attend a scheduled in-person visit and need a telehealth appointment you must provide a 24-hour notice to avoid our standard no show or late fees.  This appointment may or may not be available at the same time. 

  • Our Support Team is currently not checking in patients in-person. Upon arrival at our waiting room, please follow the instructions posted in our office on how to notify your provider you are here.
